Hearing Aid Benefit
You, your spouse and eligible dependents are covered for hearing aids.
What Is Covered And What Are The Coverage Limits?
As of September 13, 2019 you and your eligible dependents are covered up to $4,000.00 per family maximum during a 36-month period. This reimbursement covers the cost of the hearing aid, batteries, and repair.
What is the claim procedure?
1. Send a copy of a Doctors note certifying a medical necessity.
2. Send originial paid bill.
3. You will be reimbursed for an amount not to exceed $4,000.00.
